Eligible persons

Voluntarily and Continuously Insured Persons and dependent spouses, and other dependents aged 35 and older, who are members of the Health Insurance Society as of April 1
* Those who no longer qualify as dependents or who have lost their eligibility as Voluntarily and Continuously Insured Persons as of the exam date are ineligible. Note that ineligible persons who have undergone exams must pay the full costs of the exams themselves.

The health exam subcontractor Wellness Communications will send a Guide to Health Examinations to your home address around the end of May.

･ Read the guide, then apply for an examination.
　　　↓
･ Get examined at the examining institution.

Notes on health exams

①Please make an appointment as soon as possible when you have a gastric endoscopy.

②Address inquiries to Wellness Communications
Address any inquiries concerning matters ranging from applying for to getting an examination to the Wellness Communications Health Examinations Appointment Center.

③Make your copayment at the medical care institution on the day of the examination
Make the copayment for Chest CT scan at the counter of the examining institution on the day of the examination.

④Subsidies are not available after loss of eligibility
Even if you were eligible for an examination at the time of appointment, if you lost your eligibility as a member of the Shiseido Health Insurance Society by the date of the examination, you will not be able to receive any subsidies.
Note the expiration date of your member eligibility when applying or changing the exam date.
