Support smoking cessation

The Shiseido Health Insurance Society will continue to provide assistance with the cost of the outpatient smoking cessation treatment as an ongoing activity. Members are encouraged to take advantage of this program. The details are provided below.

Support program for the total costs of smoking cessation

①Outpatient smoking cessation treatment
Administered under the guidance of a physician, the outpatient smoking cessation treatment requires five consultations (examinations and associated treatment) over a 12-week period. This treatment is covered by health insurance as long as certain conditions are satisfied. With insurance coverage, the amount of copayment (30%) may vary, depending on the medicines used and the medical care institutions involved; it is typically around 20,000 yen.

②Details of assistance with smoking cessation treatment costs
Up to 5,000 yen of treatment costs will be reimbursed to those who complete an outpatient smoking cessation treatment covered by insurance under a physician’s guidance and quit smoking.

Conditions	①You must complete an outpatient smoking cessation treatment under a physician’s guidance and quit smoking. ②Retain receipts for payments you make toward the outpatient smoking cessation treatment. ③To apply for assistance with smoking cessation treatment costs, submit an Application Form for Reimbursement of Smoking Cessation Treatment Costs along with your receipts after completing five consultations.
Eligible persons	①Insured persons and dependents (family members) aged 20 or older who are members of the Shiseido Health Insurance Society from the time treatment began through the time of application ②Those who have completed an outpatient smoking cessation treatment under a physician’s guidance and quit smoking ③Those who have completed five consultations in principle ④Those who received this assistance in the past but resumed smoking and now have received an outpatient smoking cessation treatment once again
Application method	To apply, download the Application Form for Reimbursement of Smoking Cessation Treatment Costs, fill out the necessary information, attach receipts (photocopies acceptable), and send to the Shiseido Health Insurance Society (Attn.: Smoking Cessation Assistance) by in-house mail or postal mail.
Payment method for refunding costs	Remittance into your designated bank account (remitted at the end of the following month for applications received by the end of the current month)
