Acupuncture, moxibustion, massage, and shiatsu are covered by health insurance benefits (for medical care expenses) if certain conditions are met.

	[Documents to attach] The Application Form for Medical Care Expenses (original) for therapist treatment Letter of approval from an insurance doctor Receipt (original) Report on treatment (copy) (when obtaining subsequent approval)
	[If you received the treatment at home] 「Home Treatment Confirmation Form」
Deadline:	As soon as possible
Applies to:	Insured persons and dependents meeting the conditions for payment of medical care expenses and have been examined and received letters of approval from insurance doctors
Address inquiries to:	Those employed at the Head office, Shiseido Japan, JRI, THE GINZA, GIC, KODOMOLOGY, or ETWAS, or Nasu Factory, or Osaka Factory, or Ibaraki Factory, or Kurume Factory, or IPSA, or PFJ, or EFFECTIM, or Ettusais, or Shiseido Pharmaceutical, or ＳＩＢ, or Shiseido Creative should submit the required documents to the Astech Contact Desk. Others may submit them to their own HR sections. Voluntarily and Continuously Insured Persons must submit the documents directly to the Health Insurance Society.
Notes:	If continuing treatment is deemed necessary after six months (one month for manual correction of deformities), a second letter of approval from an insurance doctor must be obtained.
