Death

The Health Insurance Society will pay “Funeral Expenses” in the event of the death of an insured person or dependent. It will pay “Funeral Costs” to the person who actually administered the burial of a deceased person in the event of a deceased person with no family members or close friends.

	◎Shiseido Health Insurance Society dependents Document certifying death (Copy of one of the following: death certificate, burial permit, cremation permit)
	◎Not a Shiseido Health Insurance Society dependent, but a person whose livelihood depended on the insured’s income Document certifying death (Copy of one of the following: death certificate, burial permit, cremation permit) 《If the deceased lived with the insured》 Certificate of residence (showing that both the deceased and the claimant lived in the same household) 《If the deceased lived apart from the insured》 Copies of documents showing a regular allowance was provided
	◎Not a Shiseido Health Insurance Society dependent or a person whose livelihood did not depend on the insured’s income Document certifying death (Copy of any of the following: death certificate, burial permit, cremation permit) Receipt for funeral expenses * The receipt for funeral expenses must indicate the full name of the claimant.
Deadline:	As soon as possible
Applies to:	Shiseido Health Insurance Society dependents or persons whose livelihood depended on the insured’s income (funeral expenses) The person who actually handled the funeral, if the deceased had no dependent family members (funeral expenses)
Address inquiries to:	Those employed at the Head office, Shiseido Japan, JRI, THE GINZA, GIC, KODOMOLOGY, or ETWAS, or Nasu Factory, or Osaka Factory, or Ibaraki Factory, or Kurume Factory, or IPSA, or PFJ, or EFFECTIM, or Ettusais, or Shiseido Pharmaceutical, or ＳＩＢ, or Shiseido Creative should submit the required documents to the Astech Contact Desk. Others may submit them to their own HR sections. Voluntarily and Continuously Insured Persons must submit the documents directly to the Health Insurance Society.
Notes:	When requesting payment of Funeral Costs, attach receipts indicating the cost of the burial, in addition to the Death Certificate.

Deadline:	As soon as possible
Applies to:	Person on whom the deceased was dependent
