“A Childbirth and Childcare Lump-Sum Grant” is paid when an insured person gives birth. “A Dependents' Childbirth and Childcare Lump-Sum Grant” is paid when a dependent family member gives birth. Don't forget to add the newborn as a dependent in these cases.

Choose one of methods 1, 2, or 3 below.

When using the system of direct payment of Childbirth and Childcare Lump-sum Grant to medical institutions

Submit the documents on agreement to use the system to the medical care institution or other facility where you plan to give birth. (You need not take any action with respect to the Health Insurance Society. For details, check with the medical care institution or other facility where you plan to give birth.)

Even if you use this system, if the childbirth expenses are less than the Childbirth and Childcare Lump-Sum Grant to be paid, submit the following application to the Health Insurance Society.

	[Documents to attach] Copies of documents on agreement to use the system from the medical care institution or other facility (documents demonstrating that you have concluded a proxy agreement with the medical care institution or other facility to use the system of direct payment of Childbirth and Childcare Lump-sum Grant to medical institutions and indicating the name of the Health Insurance Society as application recipient)
	Copies of receipts for and details of childbirth expenses (Documents demonstrating that the details are the same as those on the special invoice submitted by the medical care institution or other facility to the Health Insurance Society and, if giving birth at an institution that is a member of the maternity medical care compensation scheme, showing printed or stamped text certifying that the childbirth took place at a member institution)
Deadline:	As soon as possible
Applies to:	Insured persons or dependents using the system of direct payment of Childbirth and Childcare Lump-sum Grant to medical institutions if childbirth expenses are less than the Childbirth and Childcare Lump-Sum Grant to be paid
Address inquiries to:	Those employed at the Head office, Shiseido Japan, JRI, THE GINZA, GIC, KODOMOLOGY, or ETWAS, or Nasu Factory, or Osaka Factory, or Ibaraki Factory, or Kurume Factory, or IPSA, or PFJ, or EFFECTIM, or Ettusais, or Shiseido Pharmaceutical, or ＳＩＢ, or Shiseido Creative should submit the required documents to the Astech Contact Desk. Others may submit them to their own HR sections.
Notes:	Cannot be applied for childbirth overseas.

When using the system of receipt of the Childbirth and Childcare Lump-sum Grant directly by the medical institutions on your behalf

If you would like to use the system of receipt of the Childbirth and Childcare Lump-sum Grant directly by the medical institutions on your behalf, submit the following application to the Health Insurance Society in advance.

Required documents:	「Claim for Childbirth and Childcare Lump-sum Grant (for Receipt on Your Behalf)」 (A4, 110KB) (showing the signature and seal of the medical care institution or other facility that will receive payment on your behalf and other required information) Example of completed form(114KB)
Required documents:	【Documents to attach】 Copy of maternal and child health handbook (pages indicating the name of the expectant mother and the anticipated childbirth date)
Deadline:	Within two months before the anticipated childbirth date
Applies to:	Insured persons and dependents expecting childbirth up to two months in the future who wish to use the system of receipt of the Childbirth and Childcare Lump-sum Grant directly by the medical institutions on your behalf
Address inquiries to:	Those employed at the Head office, Shiseido Japan, JRI, THE GINZA, GIC, KODOMOLOGY, or ETWAS, or Nasu Factory, or Osaka Factory, or Ibaraki Factory, or Kurume Factory, or IPSA, or PFJ, or EFFECTIM, or Ettusais, or Shiseido Pharmaceutical, or ＳＩＢ, or Shiseido Creative should submit the required documents to the Astech Contact Desk. Others may submit them to their own HR sections.
Notes	Cannot be applied for childbirth overseas. Obtain consent to receive benefits on your behalf from the medical care institution or other facility where you expect the childbirth to take place. Apply within two months before the anticipated childbirth date.

If you paid the entire amount of childbirth expenses to the medical care institution

Submit the following application to the Health Insurance Society if you have not used the system of direct payment of Childbirth and Childcare Lump-sum Grant to medical institutions or the system of receipt of the Childbirth and Childcare Lump-sum Grant directly by the medical institutions on your behalf or you have given birth overseas.

	[Documents to attach] Copies of agreements from the medical care institution or other facility (documents demonstrating that you have not concluded a proxy agreement with the medical care institution or other facility on use of the system of direct payment of Childbirth and Childcare Lump-sum Grant to medical institutions and indicating the name of the Health Insurance Society as application recipient) * Not required for overseas childbirth

Applies to:	Insured persons or dependents who have paid the entire amount of childbirth expense at the medical care institution without using the system of direct payment of Childbirth and Childcare Lump-sum Grant to medical institutions or the system of receipt of the Childbirth and Childcare Lump-sum Grant directly by the medical institutions on your behalf

Notes	If you cannot have a physician or midwife fill in the certification space of the document for overseas childbirth, you must attach a birth certificate or other documentation issued by the physician or midwife. * Attach a translation if the document is in a foreign language.

Add the child

After the child is born, take the necessary steps to add him or her as a dependent.
