After you leave your employer

You will lose your eligibility for membership in the Health Insurance Society after you leave your employer. Join the appropriate medical care insurance system based on your needs and circumstances.

Return your health insurance card when you leave your employer

Required documents:
  • Health insurance cards (for the insured person and all dependents)
  • Elderly benefits card (if issued)
  • Issuance of Maximum Co-payment Certificate for Health Insurance (if issued)
Deadline: Within five days after the date of loss of eligibility
Applies to: Insured persons leaving employment and their dependents
Address inquiries to: Those employed at the Head office, Shiseido Japan, JRI, THE GINZA, GIC, KODOMOLOGY, or ETWAS, or Nasu Factory, or Osaka Factory, or Ibaraki Factory, or Kurume Factory, or IPSA, or PFJ, or EFFECTIM, or Ettusais, or Shiseido Pharmaceutical, or SIB, or Shiseido Creative should submit the required documents to the Astech Contact Desk. Others may submit them to their own HR sections.

If you wish to remain a member of the Health Insurance Society

Required documents:
  • * In certain cases, since each month’s insurance premiums are collected in the following month, two months’ premiums may be collected in the month in which an individual leaves the company.
    If the premium amount to be paid in the month of leaving the company is twice that paid in the previous month, check the amount of the premium for a single month.
Deadline: Within 20 days after the date of loss of eligibility of the insured person
Applies to: Insured persons who have been insured continuously for at least two months prior to leaving employment

If you want your family members to remain dependents

If you want family members who were dependents while you were employed to remain dependents, fill out and submit a Notification of Health Insurance Dependent (Change) of the Notification of Eligibility As Health Insurance Voluntarily and Continuously Insured Person, together with the required certifying documents, by the deadline for membership procedures.

  • Even those who were previously certified as dependents will be screened again to confirm eligibility for dependent status.
  • In certain cases, family members who earn an income may not be certified to remain dependents.
  • You cannot apply to add new dependents (except newborn infants) when applying for the Voluntarily and Continuously Insured scheme or while you are a Voluntarily and Continuously Insured Person.

If you started a new job and become an insured person under another health insurance plan, submit the three documents indicated below to the Shiseido Health Insurance Society.
Complete similar procedures even if you are returning to the Shiseido Health Insurance Society as an insured person.

  • Notification of Loss of Eligibility As a Voluntarily and Continuously Insured Person (a document sent to you together with the information materials sent when you joined the scheme)

  • Shiseido Health Insurance Society Voluntarily and Continuously Insured Person health insurance card (original)
  • Copy of health insurance card from your new insurer

Note:  

<Refund (return) of insurance premiums after loss of eligibility>
Insurance premiums may be refunded in certain cases.
Eligible persons will be issued a refund request form following receipt of their Notification of Loss of Eligibility As a Voluntarily and Continuously Insured Person. Fill out the information required to request a refund from the Health Insurance Society.

  • * Insurance premiums for the first month of membership in the Voluntarily and Continuously Insured scheme cannot be refunded.
  • * If you joined any of the following social insurance programs in the first month of membership in the Voluntarily and Continuously Insured scheme, you must pay the premiums for each program.
  • * Note the limitations on the period during which refund requests are accepted. No refunds will be issued after two years since they first become payable. Take the specified steps as soon as possible.

You cannot cancel your status as a Voluntarily and Continuously Insured Person by switching to National Health Insurance or becoming a dependent under other health insurance.
If you use your health insurance card for examination, treatment, complete checkups, or other services after loss of eligibility, you will be billed for the full cost of such services.
